How a Trainee signs off the ESR:
Below we list what the steps a Trainee must follow to complete the ESR sign off process (this is shown at 3:48 in the video above):
1. Review your Agreed PDP & Suggest New PDP:
As part of the ESR preparation, you will need to ensure that you have reviewed all of your previously agreed PDP that you have been working on in the current Review Period (and carry over any that are incomplete) and suggest new PDP for the next Review Period.

2. Start ESR preparation
You will not need to 'create' an ESR for the Supervisor to complete, as they will have access to your current Review Period and the ESR is a built in feature on the Prepare page.
Scroll down and click on Prepare in Portfolio Summary box (below shows a trainee on the new curriculum)
3. Complete the 2 Trainee ESR forms
You will see 2 Trainee forms listed underneath the 'To be completed before sign off' area that are shown in red (as below):
Clicking on each of them will take you into the relevant form - please be aware that these are designed to be completed in one sitting.
Once you have clicked Save in each one, this submits the form and it will disappear from your 'to-do' list.
4. Supervisor completes their ESR Forms, PDP Review and Sign Off
Once you have completed your 2 forms in step 3, the rest of the sign off is done by the Supervisor and you can see what they need to do here.
5. Trainee sign off
Once the Supervisor has completed their sign off, you will receive an email notification:
You will also see on your Training Map that the Report lozenge turns green:
When you next sign into your account, you will see a notification on your Dashboard prompting you to Sign off Education Supervisor Report as shown:
You can review all the comments and then you will need to select 'Yes' or 'No' as to whether you agree with the review, add comments and then click Save:
