How to create an Organisation Patient Survey

• To begin setting up a Friends and Family Test please click on the Surveys icon from your Dashboard.
• Next you will be able to select which Survey you would like to create.
• Select My organisation and then Friends and Family test.

The setup page will now appear. Here you need to enter your practice details and select a follow-up question. You can choose from a list of pre-made questions or opt to create your own question. Once you have entered your information click on Create the Survey. 

Once you have clicked Create the Survey the questionnaire will be complete to the minimum NHS England requirements and you will be taken to an advanced setup page. 

Here you can check the details of your survey and print paper questionnaires for patients to complete.

Also on this page you can make payment for your survey, find the website and app links, email staff members instructions and a link for entering paper responses, or add new questions.
