How to use paper forms to gather feedback for the Organisation Patient Survey

When you have set up your Organisation Feedback Survey you may want to hand out some paper questionnaires to your Patients for them to complete and hand in whilst they are at the Practice.

To print off paper questionnaires

• Click on Surveys from your dashboard.
• Select the Organisation Feedback Survey you wish to print questionnaires from.
• Enter the Survey Setup by clicking on Continue next to the survey.
• Click on View and print paper questionnaires
  (on the right had side of the Survey details section).
• Here you can download a PDF of the questionnaire to print.
  
  

To enter the results from the paper questionnaires

• Go to the Organisation feedback setup page.
• Scroll to Links.
• Click on www.leavemyfeedback.com/your unique survey number.
• You will then be emailed instructions detailing how to go about entering your results.
