How to customise questions on the Organisation Patient Survey

It is possible to customise a Friends and Family Test. Therefore you will be able to create a survey that will ask the questions that are important to you as an organisation, and enable you to get the feedback in areas you are after.  

To do this

• Click on Surveys from your Dashboard.
• Now click to create a survey.

• Please select to create feedback on your organisation.

• Select Friends and Family test.

• Complete the set up page.

• Click Create the Survey.
• Scroll to the bottom of the page where you will see the opportunity to enter questions.
• Select a question type and click Add new.

• Here you can add your question and either pick a preset answer group or create a new one on the answer set page.

• If you choose to create a new answer then you will need to click on the pink answer set page highlighted in the second sentence at the top of the page. Then you will be able to + Create a new answer set.

• Here you can add the answers and save.
