Below we list the steps resident doctors must follow to complete the ESR sign off process.
1. Review your Agreed PDP & Suggest New PDP:
As part of the ESR preparation, you will need to ensure that you have reviewed all of your previously agreed PDP's that you have been working on in the current Review Period (and carry over any that are incomplete) and suggest new PDP for the next Review Period.

2. Start ESR preparation
You will not need to 'create' an ESR for the Supervisor to complete, as they will have access to your current Review Period and the ESR is a built in feature on the Prepare page.
Scroll down and click on Prepare in Portfolio Summary box (below shows a trainee on the new curriculum)
3. Complete the 2 Trainee ESR forms
You will see 2 Trainee forms listed underneath the 'To be completed before sign off' area that are shown in red (as below):
Clicking on each of them will take you into the relevant form - please be aware that these are called forms but are confirmatory statements with a box to tick and agreement button.
Once you have clicked Save in each one, this submits the form and it will disappear from your 'to-do' list.
4. Supervisor completes their ESR Forms, PDP Review and Sign Off
Once you have completed your 2 forms in step 3, the rest of the sign off is done by the Supervisor and you can see what they need to do here.
5. Resident doctor sign off
Once the Supervisor has completed their sign off, you will receive an email notification:
and get a notification on your Dashboard page when you next log in:
Follow this link to review the report in full. At the bottom of the report page there is a box you must complete yes or no to agree, add any comments the ESR and sign it off by clicking Save:
6. After sign off
You will see on your Training Map that the Report lozenge turns green and in the information box you can see the ESR status and supervisor sign off date, your agreement status and any comments:
When you click View the Review Period will still open for you to view but not edit, and there is a banner at the top of the page highlighting This Review Period is in the past (Supervisor has signed it off but not resident doctor) or This review periods is now locked (signed off by Supervisor and resident doctor):
At this stage, if your Supervisor has not yes, you need to add a new review period and link your Supervisor:
