This message will appear if the Profession that you have set and the appraisal template you are using is conflicted. For example: if you are a GP and you have a Non-Medical appraisal template set.
Depending on what status your appraisal is at will depend on the steps necessary to change the template:
My Appraisal has not been sent to the Appraiser:
Save a copy of your current appraisal
Make sure you save a copy of your appraisal in its current template before making any changes.
- Go to your Portfolio page
- Click on Appraisal Summary (you will need an active appraisal subscription before doing this):
- Download a pdf of the appraisal summary by clicking on Without attachments as shown (this is added to your device downloads folder):
Check/Change your Profession & Appraisal Template
- Go back to your Portfolio page
- In your address bar, edit the URL from https://www.fourteenfish.com/appraisal/overview to https://www.fourteenfish.com/appraisal/change as shown:
- Check that your Profession is correct (this is important so the system offers the right Appraisal Template for your role) - if it needs correcting, click on Change this (as shown above)
- Once you have the correct Profession listed, you can change the appraisal template to the suggested one by clicking Change this
Check/Amend your Appraisal
- Once you have changed the appraisal template, your screen will change and you'll be notified of any missing areas in your new template
- You will also be advised of any information that has been added as an 'Additional Information' entry on your All entries page titled 'Information we could not transfer from your appraisal toolkit'.
- If just the Probity statements need changing, you will just need to re-enter those
- For any other information, you can use the saved appraisal summary you downloaded to copy/ paste the details into the correct fields. (Depending on the templates that are switched between, the amount of information that cannot be transferred differs)
- Please make a final check of your new template compared to your downloaded summary. We recommend clicking on Send to appraiser to view ay incomplete areas
My Appraisal has been sent to the Appraiser (but not been signed off)
- Firstly, please contact your Appraiser and ask them to copy all of their notes (if they have made any) onto a Word document so that they are not lost
- Let us know when this has been done as we will need to retract your Appraisal from your Appraiser
- We will contact you once we have retracted the appraisal
- Please then follow the steps as above
My Appraisal has been fully signed off / locked
