Once an ESR has been signed off by your supervisor it is considered 'locked' to further changes. The ESR is locked even if the trainee has not yet signed it off from their side.
Adding further entries to a locked review period is not considered reason for an ESR to be unlocked.
If any further entries need to be added to an ESR, we recommend that they are added to the next open review period, and that an Educator’s note is raised by the supervisor on the trainee’s portfolio to explain the situation. Please be reassured that Educator's notes on trainee's portfolios are a common occurrence to explain missing or extra information, and this will help to avoid any confusion in the future.
If you are an ST3 trainee, and the locked ESR is your final one, we would recommend adding your remaining entries to a 'mop-up' review period. Mop-up review periods are set up as normal, but aren't signed off, and they are used specifically for trainees who have finished their last review period but still need to add entries.
