These should make up most entries and provide most of the linkage to the Capabilities and Clinical Experience Groups. Clinical Encounters and Professional Conversations will be mapped to this new entry; the clinical learning from acute, emergency or unscheduled care experiences is entered here. There is now the option within the Clinical Case Reviews to share the setting of the learning, to enable Trainees to document learning in a variety of settings (both in and out of standard GP hours, in addition to the type of consultations). Trainees are expected, after a brief description of the case, to reflect on what they need to maintain, improve or stop.
Before considering this though it is recommended that Trainees reflect on what went well and why, so that they can maintain these behaviours. This reflection should include actions needed in response to the Trainee’s emotional needs as well as clinical and educational actions i.e. “how did it make you feel?”
The option for the Supervisor to comment on the review will be retained and whenever the Supervisor disagrees with a suggested Capability or population link, the Supervisor’s explanation will also go into the comment section. As detailed below...
Out of hours/unscheduled care training requirements vary throughout the United Kingdom. Trainees are expected to document some Clinical Case Reviews of learning within unscheduled care/OOH (highlighting the appropriate setting). It is not mandatory that Trainees complete a Clinical Case Review in the ePortfolio for each OOH/Unscheduled Care Session undertaken. However, it would be expected that Trainees use a variety of evidence to demonstrate competence in unscheduled care (both Learning Logs and WPBA).
