This article relates to GPs needing to complete surveys for appraisal and how to get your responses sent to us for our data entry team.
Scan & Upload your PSQ responses:
You can scan and upload your completed surveys via your FourteenFish account, saving time and postage costs.
To do this click into Surveys from the top five menu tabs.
- You then need to click on Continue next to the Patient Survey.
- Scroll down the page and click on Upload your surveys.
- Simply follow the steps on the page you are taken to making sure hit Submit and you will get an automated email confirming that we have received your responses.
Please be aware that our uploader can only accept one file at a time!

We recommend sending your completed surveys via recorded delivery as there have been rare occurrences of surveys getting lost in the post.
Please don't use staples, envelopes, paper clips or fold the paper. We scan the responses and removal of staples etc can delay entry. If you do use staples please remove them before sending your surveys to us.
We regret we cannot return the patient surveys to you. You will be notified when we receive the responses by email and you will get a further email once the summary is ready.
We keep a copy of the patient survey responses for 6 months before destroying them securely.

This article doesnt say how to upload them via my account. If I had seen that I would not now be looking....not very helpful . Advice needed asap!

I uploaded two pages of the patient survey, confused me showing the single page number when I uploaded the second page, then I said submit and now I am unable to upload any more. Question is I need access to upload my other patient surveys, Also when we upload does it show the file name u[loaded each time. Please advise. Do I wait till I reach the minimum number of patient surveys before I upload. Do I upload all at once.

would you be able to send me the guidance too.
I have read the guidance but I am still confused
does this mean that Trainees / Practices should scan & send PSQs to you for uploading instead of the local Deanery Team (us) inputting manually
I note that there is a charge if trainees send to you
please advise on standard practice, I don't think PSQs were mentioned on last weeks walk through
I've received a number of trainee queries where there appears to be a backlog of forms to be input when located, possibly lost in the post

Hi Sarah,
Thank you for your query. The advice in this article is in relation to the PSQ for GP appraisal, not GP Trainees. I have updated the article to hopefully make the relevance clearer.
The RCGP Trainee Portfolio has the functionality for Trainees, or Admin staff, to upload paper survey responses from patients, from a weblink on the survey set up page. A Trainee will be able to share this with you, should you need it, as it is unique to each Trainee.
There is no charge for GP Trainees to complete a PSQ, the fee referenced for surveys is for qualified GPs.
